Kind of hard to tell with only one picture but I’d say HTN, or at least HTN total SMV if your height is good. You seem to have similar facial structure as me in the sense that you have a good eye area and good mandible development, but you’re being hindered by lack of projection in the maxilla/midface flatness. Surgery isn’t a necessity for you, but yes, bimax or LeFort would be an ascension.

I doubt Bimax/Lefort would ascend him significantly, although I haven't seen his side.

Yes It would, or also other methods for increasing midface projection/the appearance of midface projection like fillers, implants, rhinoplasty, etcetera. I say this because he looks a lot like me but with less dimorphism and a different nose/phenotype.

Look at his eye area and mandible/ramus, they are pretty well developed but then you also see flatness in the midface and lacking undereye support.

Yes I do agree, but Bimax / LeFort will not fix the underlying problem. Infraorbital malar implants to increase fullness in the midface is the best solution.

Proposing Bimax / LeFort to men who aren't clinically recessed is a running meme on here at this point, just look at the before / after's of men on here that get LeFort performed despite not being recessed. It does little to tackle the actual underlying problem
