Recessed Maxilla and infraorbital rim - Solutions and surgeons

Since I have a deathtier maxilla and recessed infraorbital rims, I want either to undergo...

a high cut lefort 1 for midface advancement + infraorbital rim implant

or

a quadrangular lefort 1 or modified lefort 3.

Which surgeon is best suitied for either of these surgeries? My jaw itself isn recessed, its just my maxilla and my infraorbital rim area that is dogshit.

I think for a high cut lefort 1, the best options would be Pagoni, Ramieri, Defranc, Zarrinbal? Zarrinbal is conservative as fuck and doesnt do shit. Ramieri seems to be good surgeon, I dont know if he does a high cut lefort.

Modified lefort 3 is only done by Dr. sinn. Quadrangular lefort is only done by some doctors in Russia and some crazy curry doctors I guess.

How is modified lf3 different from normal one? I am also recessed and currently looking for surgery options.

Yeah definitely go to Sinn or Sailer and have an 80 year old give you a lefort 3. The more senile and decrepit the better. If they actually fall asleep or die during the procedure you know the results will be good.

Legit, like what is even a "high cut Lefort 1"? I thought it was the same as a Quadrangular Lefort 1 jfl, but OP is making a distinction between these two.

Just different cuts. There are more than a dozen cuts of lefort 1 but these are the main ones

A) Conventional Le Fort I
B) Modified quadrangular Le Fort I
C) Quadrangular Le Fort I
D) Modified Le Fort I
E) Extended Le Fort I

Usually what users here refer to as high cut lefort 1 is C) but I'm not a fan of any of these lefort 1 variants tbh Modified lefort 2 and modified lefort 3 mogs all

Precisely,less implant material and I read somewhere that MLF2 is easier to perform

MLF3 (orbital-zygomatic advancement osteotomy) followed by traditional Lefort I + BSSO + genioplasty to fix the jaws.

You will need two separate surgeries 6-9 months apart because each complex generally has to be mobilized in a different direction and any simultaneous fixation would be quite unstable.

U will need midface implants after that which defeats the purpose of that surgery (other than reducing the implant material)

Nope mlf 2 mogs. People with recessed upper maxillas often have recessed Infras aswell

I had a consult and asked about the aesthetic results of high cuts lefort 1 or 2 for and got told the problem is that if you push the mid area forward it will make your midface looks puffy. Also nose gets bigger and wider.Got told you want mass at the cheekbones as a male not necessary at the midface. That's the risk you take with high cuts. Thats why implants or ZSO is needed/ necessary.

where did you get those pics ? I have some maxfac books but they don't contain those variations.
