It's clear I have recessed lower third.

What options do I have?

you are a pretty standard case of recession, just go to a good jaw surgeon and they will to their best to fix you up.
Just focus on getting around 10-15k to cover all the costs and they will fix you

So I saw a highly recommended surgeon where I live who suggested a 10mm horizontal advancement with sliding genio as the best first course of action rather than more extensive jaw work. Thoughts @tincelw ?

Ortognatica Roma does bone’s grafting taking it from you’r illisc crest to fill bigger gaps.
Imo 15mm it’s alot but let’s be honest generally people need 2 cm+ And i think this might be the case.
To conclude always listen to those doctors and see the simulation.

My surgeon said bimax would be overkill and genio would achieve 80% of the result with <10% of the pain/hassle.
Especially because I had no dental issues , good occlusion.
A good trade off for now.

and when you say "80% of the result", what result was he talking about, like did you show him an ideal jaw that you wanted?

No, I walked in and he said 'you need 10mm' - we went from there.
That's the max you can get with genio.

Bimax is a big deal and UK surgeons are reluctant to recommend it for someone like me with no dental issues.
Sure, you can go abroad, find someone else whatever, but you're adding risk.
I got a pretty good outcome with almost zero risk.

Sadly my maxilla is receded
So I need bimax but I think the risk/money isnt worth it in my case
 
This a bad results, not because you didn't improve but because you needed a Bimax clearly

Your surgeon coped hard on this one

You almost have lip incompetence translating into class 2, u needed braces for decompensation then Bimax

It's not because u improved that it was a good thing , you are still candidate for Bimax

I will not deny u gain in term of PSL etc but man Bimax would of given you way more drastic and better results
 
sure, I'm still a candidate for bimax (in some surgeons opinion) but that's a much bigger deal.
if I do bimax , at least I will have also had genio for extra mm.

I'm looking at braces first. Invisalign maybe
