As the rumors say "Axl went off public view for a while, then returned another person" most people said he got a fail plastic surgery, but what exactly happened to him ?

Can you guys explain what happened to his face ? is it surgery or normal aging ? if it's normal aging, what could cause the face to get so swollen and bloated permanently ? because I cannot find what.
