Too few grafts, it's not as dense as her regular hair (you can see it behind the transplanted hair) also hairline needs to be more irregular

styling your hair up and back. think about it logically, the density will never be 1:1 like in a non balding person, plus the grafts can be seen in such a close up and lightning

the hairs were put too far apart from each other (few grafts used) and they used grafts with thick hair and multiple follicles within. Hairline hair should be thinner, progressively getting thicker and denser

Bad work on a hairline. It should get the most effort as it's the most visible part, and you can even sacrifice mid part density to some degree as long as you get a decent hairline. That's why body hair almost never used for a hairline. One may get it "too artificial" though, need a surgeon with an eye for aesthetics.
