Look how wide are airways in horizontally growth mandible , ramus is mostly responsible for narrowing airways so funcionally ramus should be the most important factor during jaw surgery. It should be an argument to get some ramus osteotomy.

Rotation+advancement anteriorly in the lower jaw+custom midface implants+angle augmentation with chin wing or a jaw angle implants.

This is possible for most people and results are mind blowing the biggest change should be the mid face your maxilla is by far the most important bone in your body

So brutal maxfac will never understand importance of horizontal growth

Only who understand is Mew but he is almost like pseudoscience

It's interesting how you hardly need any maxillary forward movement at all in order to improve the airway. It is practically enough to simply shorten the maxilla, after which the mandible is free to rotate away from the throat.

Sphenoid bone is a huge bottleneck but very little you can do, just decade of tongue posture (since theres not really a looksmax surgery for it) so theres almost no point in talking about it.

Yeah I know that downward growth can't be treated with surgery
And all you can do is posture, tongue , chewing and lip seal , still better than nothing
