Weird old guy on the train. Wtf is wrong with him?

Some old guy wearing a long coat and hat sat on the seats facing the opposite side of me and was holding something wrapped in cloth on his lap

During the ride he starting moving his leg up and down while tapping his feet repeatedly. He was also slightly rocking back and forth

After a few minutes of this he closed his eyes and opened his mouth, before letting out a quite moan

He then got up and stumbled out of the train. I have no idea what he was on. What the hell was his problem?
