Went in for a Bichectomy consult to get HOLLOW CHEEKS, got recommended HA Fillers instead. Thoughts?

I need some advice. I went to a local clinic (Dorsia in Spain) specifically asking for a Bichectomy (Buccal Fat Removal). My main goal is to achieve hollow cheeks and a leaner, more defined face.

However, the consultant steered me away from surgery and suggested that balancing my features with Hyaluronic Acid (HA) would give me better results than removing fat. She claims that adding structure is better for the "hollow" look than emptying the cheeks.

She gave me two different quotes valid for a couple of days. I want to know if this protocol makes sense for getting hollow cheeks or if I'm just being upsold on fillers.

Here are the details from the papers:

Option 1: Cheekbone Augmentation

  • Goal: Harmonization of cheekbones and cheeks to improve sagging and aesthetic balance.
  • Quantity: 2 Vials of Hyaluronic Acid.
  • Price: Offered at 575€ (approx. $605 USD).

Option 2: Facial Flaccidity & Volume

  • Goal: Repositioning of volumes. Can treat areas like Nose, Cheekbones, Jaw & Chin, Nasolabial folds, or Lips.

The Question: Can adding volume to the cheekbones/jaw really create a "hollow cheek" effect visually, or is the bichectomy strictly necessary for that? Is 3 vials enough to make that difference?

Honestly, take a look at the doctors there, if they themselves dont look botched they are probably right lol. But on a deeper lvl, yes, hollow cheeks look nice, but mainly cause they show of your well developed bone, if there is bad underlying structural developement, hollow cheeks are gonna be less of an asecension then added mass. If filler is placed correctly and has the right density it can do more good then alot of people on here claim. I would be careful with cheek filler though as migration is common there, jaw filler is generally much safer and longer lasting.

When we talk about hollow cheeks, we mainly just speak about a good ogee curve, with good facial depth. The ogee curve is made up of pertruding cheeks and your chin area contrasting the jaw. Also, while buccal fat removal can be helpful, I would only advice it, if you clearly hold aboce average ammounts of fat in your cheeks. The buccal fat pad is needed to keep skin looking healthy as u age and a completly reduced buccal fat pad might just make your face worse faster then nessecary, we are still in the process of researching that.

Honestly, No. Fillers are overhated on this forum, but bad fillers especially in the cheek area will migrate 100 percent. How ever, jaw fillers and under eye fillers can be super benefitial.

actually agree, I would never get high volume fillers done in the cheek area.

Saying you need hollow cheeks is a grey statement tbh. Projection of your zygos and chin is what creates the look most ppl want. If you arent fat (which would just mean you need to lose weight, not get a medical procedure) or hold above average ammounts of buccal fat you shouldnt considre fat removal imo.

actually agree, I would never get high volume fillers done in the cheek area.
Low volume with ultra high density fillers or mixed fillers with lower hyaluronic acid content, maybe, but honestly implants will be the best option if you are severly lacking, if you are just slightly deficiant and dont have endless money to waste on MSE to widen the bizygomatic width or to get implants, I would stay away from altering my zygos and compensate with an improved jaw line. I do believe that some compensation is possible in this area. Usually I would say a bad feature cant be overshadowed by another very good one, but in this case, as pertruding zygos are mainly good for forming a good ogee curve, I belive that a good jaw can compensate to some degree as it also help the ogee curve to stand out, creating a nice backdrop and adding depth-.

In the zygo area, yes it will most likely, but more then migrate, it will be dissolved by your bodies natural uptake of hyaluronic acid which isnt bad, just means it will fade. The jaw area is not known to have significant filler migration.
