I'd say your IPD and narrow jaw are your biggest flaws but i would not fuck around with IPD for surgery.

Skin is also another flaw but that's an easy fix.

You would definitely benefit from jaw implants, for sure

You are by no means ugly at all, idk what your height is but you shouldn't struggle hard off looks alone.

You'd be hot to ethnic women if that's your taste.

Problem is, you are in this grey area where a LOT of guys are where they don't look ugly, have some good features etc, but lack appeal. Appeal is heavily important. You can try grow your hair and see what suits you.
