What movements should I request the surgeon to perform? [Trimax]

I'm about to consult a surgeon for Trimax and I don't really know what kind of movements I should insist, since surgeons often have different approaches. I'm obviously recessed af (especially on the right side of my face) with a small lower third, a subhuman concave shit profile when I laugh making my nose droop down and and the chin kinda go up. Would downgrafting be better than cw rotation? Idfk hopefully you can help me out
