Try acnatac it has antibiotics + tret it helped me when I had active acne

Isotretinoin works better of course, but since your brother doesn't want to do that, you should know that tretinoin is 50/50. Changes some peoples lives, does nothing for others. It did absolutely nothing for my acne, but it's all anecdotal. Look into how much he should apply and to what concentration, how he reacts to it is genetic.

you’re using topical tret or isotret (accutane) there’s lots of dryness issues. I know mainly only abt tret. Stay out of the sun when you can, where sunblock always outside, always moisturise.

start with 1-2x a week for 2 weeks then do every other day for 2 weeks and then do every day. If u get irritation on every day do sandwich method (moisturise before and after applying) and cut back the dose to eod for a bit until ur skin gets used to it more. This is what worked for me
