I have an appointment with my dermatologist because I've been struggling with severe acne for a long time. It doesn't go away, no matter what I try. I've improved my diet, I drink enough water, and I follow a consistent skincare routine. I've tried many over-the-counter creams and treatments, but nothing has made a significant difference.
My acne is persistent and is affecting my confidence and mental well being. Because other treatments haven't worked, I would like to discuss starting Accutane (isotretinoin). I feel like it might be the right option for me at this point.

Tell your derma exactly what you told us. Everyone's skin responds differently to different things.

not that difficult to get prescribed accutane, got prescribed oral accutane alongside a creme not even a minute into my derma appointment

just explain you have problems with your acne on your back and chest aswell as the face and you should get both oral accutane and a sort of creme/gel to apply topically

Pretty simple, just say everything you said here but to the Doctor.

Be genuine and clear, say you researched treatments and that you considered/want to start with Isotretinoin now.
If you want to be certain, tell him that you´ve tried other medications and that none have cleared your acne or solved your problem (tell him that you have tried Azelaic acid, Retinol, Adapalene, Doxycycline,...)

He will most likely start you on either 20mg Isotretinoin or Tretinoin (topical), but be clear and say that you would rather have the oral version, you will most likely have to take a blood test after a month or two, just to check if everything is in order as Isotretinoin is not harmless and does come with some side-effects.
