It’s anterior maxillary impaction. Also he could firstly try mse + face mask for better result so mse would widen his maxilla as possible and face mask could drag whole maxilla forward a little (it won’t make a lot difference tbh but still better). Now I doubt it’s safe in his case, that’s why result decent but he turn from shit to normie. I bet initial problem was his was narrow palate so he couldn’t fit his tongue for proper posture as a kid that’s why his face is so long and narrow with lack of depth.

Ideal scenario here was:
1. Mse + fm
2. Maxillary impaction
3. CT-Bone (but I’m not aware if it’s available tbh, so maybe titanium) implants for the cheekbones and undereye area) but it’s optional as upper maxilla isn’t that flat tbh.

At least I’m looking as this guy pre op (just not that narrow and long faced) so that what I would try I guess, but all that stuff cost ton of money, bruh
