Title, on 30mg accutane, no active acne just scarring and PIE. Whats the best solution
 
Send a photo of the scarring please.
You can blur your face/zoom into where the scarring is, just make sure the photo is HD.

@chudacension You still have a bit of acne. Stay on the isotretinoin.
Regarding the scars, it's hard to tell definitively based off this imagery but it seems like you have primarily rolling scars.
I'd start with microneedling (use a pen, start with 1.75mm and then go to 2.5mm), either DIY with 10%+ lidocaine or go to a clinic for it.
You'll be red for 3 days, and you'll need to do it every 8 weeks.

Isotretinoin is also shown in the literature to help with rolling scars/mild atrophic scars, so stay on it regardless of acne presence. It is not a contraindication to microneedling but is for some laser. You might want to use genesis laser for PIE if microneedling doesn't help. Though, isotretinoin is not a contraindication to genesis laser specifically, and to be honest, I was on it for my Fraxis and will be on it for my Fraxel.

mirin, should i start microneedling whilst on isotretinoin or wait?

Yes.
You can do microneedling and genesis on isotretinoin.
You shouldn't do CO2 laser on isotretinoin but I did/will anyway.

also aids, is there anything else i should do for acne scars? maybe a retinoid?
