What's the point of going to a "blackpilled surgeon"

If the technique is the same?

Why I can't just go to some cheap orthodontic surgeon for 8-15k and just ask them for specific advancements?

You just have to know exactly what you need so the results are what you actually need

because you don't know jack shit about your needed movements even if you think you do
