If I remember correctly he was either a neurosurgeon or an orthopaedic surgeon, not a plastic surgeon, so he's irrelevant to the question.

Back on topic, plastic surgeons don't get plastic surgery because:
a)They're old, so looksmaxxing has diminishing returns at that point. By the time they are surgeons and start making good money, they're 35+. That's close to middle age and past your physical prime. Also, most of them are already married with kids at that point, so the desire to look good and looksmax is much lower as well.
b)They have money and social status and can get hot trophy wives if they are not significantly below average in looks. I've mentioned it before, but I have a couple of mediocre looking doctors with hot wives in my gym.
