Why does my chest look so fucked?

Do I have caved in chest (pectus excavatum) and/or gyno? My nipples are always pointy and perky and seen through any clothing. Under them there’s like a meaty-feeling substance, i don’t know if that’s fat, gyno or both. I also have APT idk if that’s a cause
