Why was Richard Ramirez so skinny?

My question is since RiRa had such high testosterone 1950 nanogram per decilitre measured, why was he so skinny I get he was malnourished but even when he was in prison he still looked thin and also when he was a teen friends described him as skinny, even Richi himself was aware of this as he was reported to be extremely insecure about his weight as he thought it was to low
