he's saying that if you already have full lips then mouth widening is not recommended since it'll cause an off-ness to your lips

if your lips were slimmer than mouth widening would be fine since it'll actually make your lips look naturally plumpier

it makes your lips bigger because it stretches your lips which plumps out in an outward motion

you can try it yourself, take your hand, place it on your cheeks and stretch
you notice your lips plump outward making it slightly bigger

now this isnt 100% accurate but there is a similar effect with mouth widening

it's surgical to widen my lips and make them more harmonious with my face, basically improving ratios

The additional width from the surgery will just be a small crease it wont have the plump texture like the rest of your lips so the stark difference will look off I assume
