His biggest falio is that he's too fat. Most people underestimate how little extra fat makes your face not look good. The extra belly fat also messes up posture, which causes the lower jaw to be pulled backwards in naturally. This sets off all sorts of systemic problems with looks and health such as mouth breathing. Air is filtered much better through the nose than the mouth, so mouth breathing leads to more internal damage from pathogens that slip through your exterior natural defenses. You immune system will also unleash all sorts of attacks to destroy those viruses inside your body that will further damage all your organs, and even your brain. All this leads to a more sickly less attractive look.

But mouth breathing (in this case caused by posture caused by belly fat) also occurs while sleeping. This causes you to have a dry mouth and have to get up to use the bathroom more often while sleeping which disrupts sleep quality which means your body isn't fully healing which leads to a much less attractive look.

Another issue is that because the mouth is not fully closed as often the mid face begins to droop down over time do to wolf's law. And we are hardwired to see this as a sign of poor long term health and lack of attraction. In Tom's case I do not believe he's had this issue for long enough to see any significant problem here but if he doesn't loose the belly fat he will deteriorate in this way as well over the next couple decades.

Another issue we see caused by the jaw misalignment is the folds under the mouth on the sides, which are more likely to fold because the skin is older as well. These folds are very deep and within them and under them I believe there is some extra damage that is caused by drooling while sleeping, which again is caused by the jaw misalignment. This causes what's called a drool rash because the drool irritates the skin. This can lead to scaring and thickening of that area of skin, and presumable hyper aging.

As far as skin thickness, my hypothesis is that damage to skin via anti-wrinkle tools such as Drma stamp, lasers, glycolic peels, etc. also causes the skin to become thicker and more rubbery. Another possibility for the cause is not the damage but over stimulating collagen production. There's a balance for optimum looks results but in general I suspect it's best to do these sort of things significantly more infrequently than often advice.

There is also a growth on his skin on his lower cheek near his nose. This was in the pictures when he was younger but it has continued to grow. He could possible get it removed with surgery but there will be a gaping crater in his face after he removes it. I suspect the crater would look better.
