I got back home from Italy yesterday, there is still swelling frontal wise, but in terms of side profile I think that's how it's going to look permanently, maybe the chin will be slightly less taller, overall im very happy with the result and will keep updating and post frontal once the swelling is gone

looks very good, but again the swelling will exaggerate the projection a bit.

I don't think so maybe making the chin taller, because I touch the tip of the chin and there Is almost to no fat / tissue there, meaning from what i see on the side profile is bone, but again you might be right, I can only wait and see

You can see the step off where the cut was performed - not that deep but this is one of the few cases where the gonial angle can be increased slightly as it has moved the menton downwards

What does step off mean, I'm kinda confused on what to focus here
Because the chin was made taller, his jaw and chin might not have a smooth transition. I circled the part where it might be.

As pointed out in the post right above mine, the "step-off" is the notch where the bone has been cut, which creates a transition between the mandible and the chin that is not as straight and seamless as before surgery.
If it becomes too noticeable for your taste (after fat loss), you could probably camouflage it with a small amount of fillers (Radiesse and the likes).

While we're at it :

Your chin to philtrum ratio went from 2.0 to 2.44.
While your jaw frontal angle went from ~100° to ~87°.

Idk if that ratio is good or not, but I'm not really judging anything or taking it as a result, I will wait atleast 2 months post operation, and will update also, then I will judge

Yes, it's good.
The ideal range is 2.0-2.5. So you were already within that range before surgery, and you've just moved up towards the upper end, which usually translates into a more masculine and dimorphic look.

tbh I think 2.4-2.8 something like that. Also this ratio is tricky because you can have very lacking lower third and chin height but a 2.0 ratio because your philtrum is also short. In that case I think it tends to be better to increase chin height and have a slightly higher than ideal ratio in exchange for not having a weak lower third which is a worse failo.

i also got genio and am worried about the stepoff but i was thinking can 0.5 ml of filler fix that thoughts?

Keep in mind that was 3 dimensional genioplasty, not just sliding, meaning height , width and projection were altered not only projection

Running ghk cu currently, will wait maybe 2-3 months to see if it has any effect on the skin, if not I'm going with accutane

ghk-cu isn’t going to fix your skin. you need to either be eating properly, using topical retinoids or accutane. ghk is going to be a huge waste of time imo

Now sure you ARE recessed and the "optimal" path would have been bimax BUT if you factor in the cost, the danger, the pain, the life long complications compared to Genio then it's not even worth it in the slightest. You got 70% of the improvement with 5% the risk. Good stuff brother, I am in a similar boat to you and will be getting genio too.

I have never consulted with ram only pagnoni , honestly trust your instinct , thats what i did with pagnoni , there were alot of botch stories recently by him , but i trusted him with this genio

I feel safe w Ram but I don’t know his experience with 3D genio. Have an upcoming consult in June I’ll ask him

One thing that you should take into consideration is how will that tissue react to the chin widening and the limit of the widening, if you'd look at the edges you'll see that the chin is wider than the lowe body of the jaw by few mm's.

It would need a smooth transition.

You have to discuss it with Ram, I really don't have good knowledge about this stuff, book also a consultation with Pagnoni is my recommendation, try to get as much info as possible

Perhaps a bit of filler might suffice

Probably the best use for it vs trying to recreate what should really be reserved for osteotomies / implants

Lol almost all half decent surgeons can do a 3D genio. But how good they are at it on the other hand is another conversation
