[19M] Just got my CBCT scans. I have an overbite and very crooked teeth. I’m planning to get DJS along with rhinoplasty. 180cm, 65+-kg, 20-22% bf

My surgeon told me that we’ll start with an MSE for a slight width increase, then I’ll have braces for about 1.5 years before the surgery. I’m hoping someone can help me understand the best surgery plan, point out what’s wrong, and suggest what questions I should ask my surgeon. We haven’t discussed the detailed plan yet.

How much advancement / impaction / rotation would be best for me?
Will I need a genio on top of DJS?

Kind of unrelated, but do I have nct/pct? On the mirror, it looks like I have a slight pct, but in some pictures I take, it looks like a nct. Is it because of lens distortion?

And btw how much I'm risking if I decide to do rhinoplasty after MSE, but before djs? Just want to fix my nose as fast as possible tbh

@LEFORT17 help this brah

surgery related depends on movements and how much ur bites gonna get decompensated with braces. your chin bone aint flat and its a jaw problem since u have a class 2 bite. do you have a gummy smile, if so, you can/should get impaction. thats just some basic information anyone here with decent knowledge about jaws can you give too. i dont know your sna/snb degree, which basically defines the position of your jaws in relation to your anatomy. im guessing your maxilla (upperjaw) is slightly recessed, since theres a bump on your nose, which could be genetics or is a sign of maxilla recession. Solution: double jaw surgery, dont get fouled with lower jaw only.

Thank you for the information. I do have a slightly gummy smile. One of the things I’m also concerned about is whether the MSE will give my face a “balloon” appearance. From what I understand, MSE expands the maxilla and can make the cheekbone area wider, and I’m not sure how much it will affect my facial width since my cheekbone area is already (I think) pretty wide.

So these are my ceph measurements (SNA, SNB, ANB, etc.). Below is the morph my surgeon made.

Would this be a good ascension if I go through with it? Keep in mind I’m also planning a rhinoplasty after the jaw surgery.

ur asian right? morph looks okay, maybe a bit more of a genio. do you know your movements?
Azerbaijani. He told me I don’t need genioplasty, just double jaw surgery with rotation. Unfortunately, I don’t know the exact movements yet, probably because my teeth are really crooked right now. He said some adjustments might be made after braces. (Also, after checking all the measurements, he told me I don’t actually need MSE.)

i mean are you fine with the morph? its more of a personal preference thing, where you want your chin to be obviously

ur surgeon also wants to do a maxilla setback, since ur asian and already got a protruding upper jaw, thats fine. try to get more lower jaw/genio advancement
Yea, the maxilla part was what actually scared me a little bit, and I do agree with the lower jaw advancements. When I told him about it, he told me that it would be too much, and tbh I don't really think that this is too big of an issue? At worst, I can make my chin more masculine/attractive with the help of genioplasty

I'm mostly fine with this, just would like to get a little bit more of lower jaw advancement. I will try to negotiate this with him during our orthodontic treatment. Thanks a lot
