20mg accutane longterm or proper accutane cycle?

I usually just have acne come and go every few weeks all resulting in new scars

wondering if i should run 20mg accutane daily for 2 ish years or just do a full cycle over 6 months

The findings of this cohort study suggest that higher cumulative dosage may potentially reduce risk of acne relapse and isotretinoin retrial.(Source: Acne Relapse and Isotretinoin Retrial in Patients With Acne). 120–150 mg/kg were considered the gold standard cumulative dose, but it depends on the severity of acne (PMID: 26187395). Higher dosages reduce the risk of relapse (High-Dose Isotretinoin Treatment and the Rate of Retrial, Relapse, and Adverse Effects in Patients With Acne Vulgaris). Relapse occurred in 47.4% (CD <200mg/kg) and 26.9% (CD >220mg/kg) of patients. Higher daily doses of Accutane over 6 months were safe and effective, the cumulative dose was around 290 mg/kg, the risk of relapse was reduced to 12.5%.

yeah just slowly building it up and getting a cumulative dose of around 12k mg (im 75kg) within 6 months
aight that works then its better then just 20mgs overtime if you can handle the sides

The daily dose is of lesser importance, the main factor is the cumulative dose (Acne Relapse and Isotretinoin Retrial in Patients With Acne). In other clinical observations with a wide range of doses (from very low to high), there was no clear association with relapse if the course was prolonged after clinical improvement.

The classic protocol among dermatologists: start with a low dosage, increase it with the aim of reaching a cumulative dose of 120–150 mg/kg. It works but… Different studies show different figures, but it can be concluded that the daily dosage does not play such a big role, and that a high cumulative dose (220–290 mg/kg) reduces the risk of relapse. If you add a topical retinoid to your routine after an Accutane course, we reduce the risk of relapse to a minimum…

So:Choose your dose individually (you can increase it or stay at the same dose to lower side effects tbh).

Track your cumulative dose (the higher, the better imho) and keep taking isotretinoin until you reach it.

Once you finish isotretinoin, switch to a topical retinoid 2–3 times per week.
