Another great although impossible idea would be to make something like a verification and males can only be males , females as females.
+ their character to look according to their irl looks level.(not same face just same looks level like there should be a cap on character looks according to your irl looks)

They should have random main character generation, including facial features and race.

They should also have a system by which NPCs treat you better based on certain variables.

The biggest one should be looks. But it shouldn't be said anywhere that this is the case. The manual could say that people treat you differently based on "reputation" or alignment or whatever.

See how long it takes people to figure out that there's a hidden variable based on something out of your control that determines your social standing. Just like in real life.
