Accutane (again) after 4 years of acne?

So all throughout high school, I’ve dealt with maybe some moderate amount of inflammatory acne. Changed diets (on a cut now) and have been on a solid skincare routine for around 3 months now. Includes cera ve cleanser and moisturizer in the shower 2x a day, tretinoin pea size and cera ve moisturizer at night, la Roche posay spf 50 bronzer liquid and benzoyl peroxide 2.5% pea size all over face during the day. No improvements whatsoever. Tried to control anything that causes inflammation in my body, and nothing. My acne has been consistently looking like this for about 3 years but it got worse a few months after my first accutane cycle. The first one was my freshman year, and it had been 4 months but I got extremely flushed all the time and it was very uncomfortable so I stopped; acne gone for a few months after and during but it came right back for some reason. Would another accutane course do the trick, or am I just doing more and more harm to my body? TLDR: acne for 4 years, tried accutane 3 years ago, had to stop because of skin issues, acne came back worse than before even on a good routine, now contemplating another cycle of it.
