i’m about to start accutane soon at 20 mg. i’ve been using tretinoin for the past year with little to no results. my skin is mostly inflamed (see pic), and i’m concerned about a harsh purge when starting accutane. what can i do to minimize the chances or severity of a purge? would antihistamines help in this case?

nah the antihistamines won’t really help because they don’t prevent the acne from the purge as much as dryness

What Tret concentration did you use and how often did you apply?

you can't really avoid the purge
they might prescribe you an anti-inflammatory for a few weeks but it won't really make a noticeable difference

Hey bro

I started purging when I started accutane. Purged for around 4 days when I remembered I could try the antihistamine method, so I got myself a few tabs and the purge stopped.

So give it a try, you might be lucky and it works for you. Purging is still better than having acne forever, so do the accutane.

Yeah seems like a good enough dose for that amount of inflammation dk why it didn’t work. Accutane will fix it though.

Hopefully. You think i should stick with 20 mg or should i up the dosage?

I used Zyrtec (Cetirizine), but it’s not recommended since it can make you sleepy.

Once I finished those I got some Loratadine and Rupatadine. None of those cause sleepiness, I don’t notice much difference between them.

In the studies where they found that antihistamines helped with purges they used Cetirizine and Loratadine if I remember correctly, but any antihistamine should theoretically work.

Ketogenic diet eradicates acne in 95% cases lol

Done it for years, wish that was the case for me.
You may try intermittent fasting alternatively, seems like your problem is overactive insulin rather than glucose spikes. When insulin and glucose are low our body literally eats inflammed cells

@Aryan Incel help him

your very unlikely to purge or have a severe purge, you already have a decent retinoid tolerance.

Take an antihistamine for one month

2000mg omega 3 AM

2000mg omega 3 PM

Start low dose isotretinoin (20mg) and work your way up depending on how you react

Dr. Dan’s Lib Balm eliminates chapped lips because it is medicated with hydrocortisone (buy the correct one).

Keep a good skincare routine w/ SPF 30+ UVA UVB PA++++

Similarly, consider investing in a heavier or better quality moisturizer

Experiment with sunscreens and moisturizers until u find a rly good product; they are all so different in how they apply to the skin and how they look

Quit the retinoid while taking isotretinoin

If you do the above, your chance of purging goes to almost zero and your experience on isotretinoin will be much better.

you can't prevent the purge, it's literally an intrinsic part of taking accutane, your skin begins to renew faster, therefore pre-existing inflammation beneath the skin will just surface much faster then fade as sebum production lowers.

for me it only lasted like 2 weeks but it was pretty bad. also do a higher dose like 40mg, 20mg is too little if you wanna get rid of acne forever

20 is very low you’ll be on it for awhile if you have bad acne, i’ve been using 80mg for past 3 months ish and my skins cleared up (only got a couple bumps and scaring)
