accutane not working

6 months into accutane and my skin got worse from the purge and has not improved. side effects are cooked so im literally fucking myself for nothing. i dont eat slop, nor drink and follow my routine every day. rn my options are:
A. Finish course in 2 months at low end of cumulative dose (~160mg/kg) and pray skin somehow fixes
B. Extend course till I hit high end of cumulative dose (~220mg/kg) or even longer
both with the potential to go on a secondary course afterwards.

currently on 60mg and have been for 2 months, was on 40mg for 2 and 20 for 2 before that.

obviously ordered from indiamart so no dermatologist.

help.

pretty strange, most people i know got a bit of results in less than 4 months, maybe try using it after eating lots of fats and keep going for some more months, if it keeps not working idk what to tell you beside jst stop taking it

every side effect i have is commonly reported by people on accutane and my cell turnover rate is noticeably high, i just get new acne as the old shit clears. plus accutane is cheap as shit to buy in india so i doubt people are bothering with fakes,

at such high doses your skin is not capable of actually healing just because it gets so insanely dry, cut the dose back to 20-30mg and see if it improves
also measure dryness and side effects, aswell as nose size. if your accutane is real it WILL cause the typical side effects and shrink your nose even while your skin isnt actually improving
other measures you can implement:

heavy moisturization: estriol cream + aquaphor
topical tretinoin in addition to 20-30mg of isotret (tazarotene > tretinoin > other retinoids)
azelaic acid 10-15% (active ingredient, do NOT overuse especially while taking isotret)

to aid the healing process of your skin on isotret, prioritize moisturization heavily and if you want to you can experiment with cope methods like ghkcu/KPV/Glutathione Injection etc. to improve regeneration and lower systemic inflammation

I can try going down back down to 40mg. Every side effect I've gotten is documented plenty by people on Accutane. I had acne on my forehead but that cleared up about 90%, its just my jaw/cheek area is completely fucked. I have a heavy moisturiser but will try the other things you said. Thanks for the help.
