I meant at subclinical level, everyone has some narcissistic traits as a survival mechanism

It's something you either inherit or learn growing up. My father is on the anti-social personality spectrum hence I got a genetic predisposition to it. I have cousins who are complete sociopaths as well because of shitty upbringing. I got exposed to violence at a young age because I lived in a barrio (Hispanic ghetto) so I saw a lot of gang-related violence as a kid and the cops would be called all the time. Eventually, I moved out to a better neighborhood, but seeing my fellow spic neighbors trying to stab stray cats and sleeping homeless people plays a toll on you and changes you while you're developing both physically and mentally.

If it's not empathy then do you think it's because you have a code of conduct? Not having a lot of empathy frees you from moral limitations, but some people still follow some logical rules such as some Hitmen not killing kids/women.
