Lefort 1 + BSSO (bimax) - advances both jaws forward, increases facial projection and airway. This is foundational.
Lefort 2 or 3 (if midface is severely recessed) - brings the entire midface forward including orbital rims. More invasive but addresses the root problem.
Zygomatic osteotomy or implants - adds lateral projection to cheekbones. Osteotomy is moving the actual bone, implants are adding material on top.
Infraorbital rim augmentation - builds out the area under the eyes to reduce hollowing and improve eye support.
Secondary/adjunct:
Genio (genioplasty) - if chin needs independent adjustment after jaw surgery
Paranasal implants - fills out the area beside the nose if still flat after Lefort
Orbital decompression (if needed) - creates more room in the orbit if eyes are too prominent relative to other features
