Any tips for time/managment and avoiding procrastination? [URGENT]

Basically title,

I have exams coming up in 4 weeks and in 6 weeks I need to perform at a music school so realistically I should be practicing/studying a couple hourse a day at least.
Instead I find myself, playing games and watching porn all day.
Are there any good time managment tips.
I've tried writing a plan/time table for each day but I never actually end up following it

I have pretty decent IQ and my academic performance has always been good.

you need discipline and unironically to just do it
boxers dont feel like going to the gym but they know if they dont then they will risk being embarrassed infront of everyone and not achieving the results they want
