If you haven't already, consult a GP and get them to prescribe you tretinoin. You don't need to see a Dermatologist. Pretty expensive but more immediate (in that you can get it now) than isotretinoin. You can also get some for much cheaper off AllDayChemist.
Let isotretinoin (accutane) be your final option if you haven't explored other avenues.
