When I have to kneel down it hurts me too, but I have slightly fucked up knees. Some girls I've been with also wanted to be given something under legs to kneel on to suck me off because it hurt them too but most did it with no problem
So I guess it's probably connected to some small differences in knees and tibia that causes pain while kneeling
