I'm 18M, I've got 2 deciduous molars and no permanent tooth understand them. Orthodontist advised me to improve alignment with braces and then proceed with titanium implants. It also causes a puffy look near those areas.

You’re missing the permanent teeth, so the baby molars stayed. Braces first set proper spacing, and then implants later replace them, and the puffiness usually improves from time to time.

What about bone density loss after implants? How do I avoid it after implants?

Minor early bone remodeling is completely normal; healthy implants preserve bone.
Good placement, correct bite, hygiene, and no smoking prevent bone loss.

Either way, don't be shy to ask your dentist.
