Doesn't matter if you have some mogger districts, if it's a slum literally three miles away. This is not first world. Otherwise a lot of "shitholes" would be. To be a first world country MOST of your country needs to have good infrastructure (transport, roads, buildings, technology etc).

First world countries are G7 countries. And even then, places like USA, UK, Canada and Germany are a league ahead of Italy, Japan.
