BSSO is for mandible. Lefort is not specially for augmenting the malar region. You're better off doing a reverse L shaped zygomatic osteotomy. They do it in South Korea.lefort 2 + bsso?

lefort 3 brings all the maxilla including the malar region forward and you can do a bsso to bring the mandible forward aswell.None of the Leforts augment the upper midface adequately relative to the lower midface and mandible.
You need to compare the amount of anterior projection of the cheekbones with the projection of the maxilla. Say for example, you have a guy with downward maxillary growth with cheekbones. For him, getting a Lefort 3 will improve the zygos but will also bring the frontal section of the maxilla forward, thereby failing to increase zygo prominence with respect to the frontal maxillary region. If your maxilla is forward enough already, you should get an osteotomy of the cheekbones (upper midface) so that they alone can be made more prominent with respect to the rest of the face. If you bring the entire face forward, you don't elevate the cheekbones with respect to the rest of the face.you said
