Genio. Dont think you need to worry about the cheekbones. If you absuloutly want to do surgery eye surgerys would make more sence. Good base tho

Genio and Malar implant for anterior projection since your midface looks flat from the side. You dont need any lateral projection tho. Also a canthoplasty since you look like you have a negative tilt
