In my country all inmates are skinny as fuck, they get food enough only to survive, they don't have energy to go around, rape or cause trouble. Lifers are also kept separately, they ironically have better conditions -are allowed to keep more personal items, have their own prison cell, and don't interact with the other inmates unless supervised by a guard, like when they go to the church, library, therapy and activity groups, etc. - they're also the least likely to cause problems.
