My wild guess is that the his lower incisors proclined too far back during dental decompensation, so the jaw would have to be advanced further to fix the occlusion, giving a dental Class 1, but in all likelihood a skeletal Class 3 malocclusion.

It's more important to look at the skeletal aspect of osteotomies using CBCTs and lateral cephs than the dental aspect which many surgeons focus on
