u need to go to ur dentist, get them to refer u to an orthodontist consultation so they can scan ur shit. Get a MT scan done and ask them to send u all their scans, and get a referral from the orthodontist to be able to get the double jaw surgery.

Then book online consultation with a good surgeon, i recommend pagoni for europe, send him over the files and explain u want asthetic function, show u know what the fuck u want or need and what ur expecting. Dont be too much of a psl autist about it and dont make it seem like u got BD.

Then he should accept u, within couple months time ur surgery will be set. It overall is a process that will take a couple months,
