Bimax Morph - Looking for thoughts and advice

I recently had my pre-surgical consultation for my bimax and was able to get some photos of the soft tissue morph. I wanted to see if anyone had some thoughts on how it looks aesthetically and how maximized it is, along with what thoughts about the surgeons reasoning for these movements.

As for what my surgeon described to me and the reasoning, first off he told me that he plans to do a 3mm advancement with the maxilla. He specifically said the reason for a more conservative advancement to the maxilla is due to my nasal labiol angle. He said it’s already too open and on the more angular side so more of an advancement would likely lead to my upper lip being too pronounced and overly filled, giving a “chipmunk” look.

Next he said he planned to do a ccw rotation for my mandible, taking the original angle from about 76 degrees to 80. He doesn’t plan on either moving it back or advancing it but rather just this ccw rotation.

I don’t know, but overall in my opinion I think it’s a bit too conservative. But from his reasoning and what fits my case, I feel like everything kind of made sense on why he specifically is keeping the movements on the more conservative side. He said that he and his team looked to a lot of different advancements and changes in angles and this ended up being the best for my case.

I feel like the main conclusion from this I can make is that made just a bimax with a lefort 1 incision won’t make too much of difference for me aesthetically, and maybe rather a lefort 2 would be required or some other procedure. Specifically, I recently watched a video from creator “FaceMetrics” about how bimax isn’t necessarily this surgery that will ascend everyone in a crazy way and rather is a building block towards a good base and other surgeries, and I think this may be the case for me.

You will be disappointed if you are seeking an aesthetic result.

I Don't have sufficient knowledge on specific movements, i would advise you to consult with another doctor and discuss movements with him as well.

If you go through with this extremely taxxing Operation, it would be a pity if you don't aswell get an aestheticly pleasing result.

East Asians are generally the most difficult to advance without compromising the aesthetics. If I'm seeing correctly they want to make your face shorter, i think it's the main thing that is bothering about this morph

Yeah I agree with you. I think the main problem with me is that with my occlusion I have both a pretty sever under and open bite (not sure if you can tell from these soft tissue pictures). And this kind of is the reason why I feel like I can’t benefit that much aesthetically since that kind of results in my mandible not being able to be advanced or setback at all and rather only just a ccw rotation. And thus, because my mandible specifically won’t be able to be advanced, that would mean advancing the maxilla any more wouldn’t be ideal. As I mentioned before I also think another big factor is my nasalabiol angle being very obtuse and open compared to a more European face having more of a right angled nasalabiol angle letting more advancement of the maxilla without risk of the “chipmunk” look and relapse. I rather think this surgery would help me aesthetically a lot more in motion as I think that’s a big problem given my current under and over bite which tbh makes me look pretty retarded. I’d like to hear your thoughts on this tho and see if you agree or think it can maybe be still maximized more. Also with the shortening of the face I do kind of see what you mean because I know that ideally a long lower third is better for males and gives a more masculine look. However, I feel like in my case my lower third is a bit too robust and overly long, especially for my phenotype of a more “soft” looking East Asian male. Again tho happy to hear any agreements or disagreements and just thoughts overall. Sorry for the long read again but please leave any thoughts

Just because the nasolabial angle is open doesn't necessarily mean you have room for advancement. You definitely don't want your lips to surpass your nose or even anywhere near that.

I would like to see the ceph tho, this would help me give more proper advice. I need to see the bite and the skeletal relationship
 
Look at the thread I posted some weeks ago about the anterior nasal spine.
 
Before and before, at minimum you need a high cut lefort 1. Ideally you’d get lefort 2 because your upper maxilla is 404

Bimax ascends you insanely if you're deformed, if you're normal then it obviously wouldn't help.

I was writing a whole thing about how I'm not sure if a lefort 1 bimax + implants could blah blah blah, but damn I checked it and this is lifefuel
