Bimax surgery result – question

I’m very curious about how my bimaxillary surgery will change my appearance. I’m scheduled for a bimax surgery, possibly a Le Fort procedure. I created an AI image of how my smile might look after the surgery, but AI is still just AI and not something you can fully rely on.

This is my current smile. As you can see, I have an underbite :

Maybe a picture doesn’t help much, so here are the notes from my doctor’s facial analysis:



”Extraoral status
Profile:
Slightly concave profile.
Normal facial height, distribution 75 (lower) – 65 – 50 mm.
Nasolabial angle greater than 90 degrees with reduced upper lip support.
Mandibular base inclination slightly horizontal.
Chin–neck distance:
50 mm
Chin–neck angle slightly open, just over 90 degrees.
Mentolabial fold:
Normal
E-line:
Lower lip: 0 mm
Upper lip: 7 mm
Frontal (en face):
Thin paranasal/infraorbital region.
Shows 0 mm of upper incisors at rest
Shows 4 mm of upper incisors and 0 mm gingiva during an active smile.

Maximum mouth opening: 55 + 2 mm.

Upper lip length: 11 mm.

Nasal width: 33/40 mm.
Competent lip closure.
Asymmetry: No.


Functional (bite physiology):
TMJs palpate without abnormalities

Masticatory muscles palpate without abnormalities.

Full and normal mandibular range of motion.

Intraoral status:
Fully dentate from 7–7 in both upper and lower jaws.

General spacing in both arches, midline diastema of 2.5 mm.

Sagittally prenormal relationship.

Vertically normal bite.

Normal transverse relationship, though anterior crossbite due to the prenormal relationship.

Overjet: −4 mm

Overbite: 2 mm

Midlines aligned in both upper and lower jaws. Chin midline corresponds with lower dental midline.

Upper occlusal plane horizontally straight.”


The question is simply whether I can achieve the smile shown in the image with bimax surgery, or if that isn’t possible.

I’m not sure. As you can see, my teeth are smaller compared to the AI image, and the smile is wider with more teeth showing. What makes you think this would be possible?

if that's true, then obviously the teeth cannot be made "bigger"
and the smile is wider with more teeth showing
but via orthodontics the arch can be widened - how much, depends on the specific case
but in most people some degree of widening is possible, of course the widening is not comparable to a specific procedure that widens the palate
if the widening you get via ortho is not enough for you, you can either split the palate and widen that (SARPE) or get a segmented lefort - that's what I'm going to do

I was mostly speaking about the position of the smile/teeth itself
I think the ai result is not unrealistic, it's not some insane 12 tooth smile, but in the "before" pic, the teeth is so hidden that it's hard to judge how wide the arch is right now

Can I ask what kind of doctor you are seeing for these concerns? I'm trying to see about getting a similar procedure, but not sure where to begin.

Your teeth aren’t necessarily small. You just have a vertical maxillary deficiency (which can and would also be fixed with bimax) + your teeth are worn down (which can be fixed with composite bonding)

VMD is the reason why your upper teeth barely show.

Quite literally the exact same position I’m in, except I have an overbite

So yes, it’s entirely possible and likely that your teeth will look that way after your procedure and composite bonding (which you should consider).

EDIT: I could be and probably am wrong now that I read what your doctor wrote. It truly does look like a vertical maxillary deficiency though; I’m surprised it isn’t one.

Can I see a picture of your side profile? Can I also see a picture of you pulling your upper lip up so I can see your upper teeth fully? Just curious

I looked at pictures online of people with VMD, and it looked quite similar to my problem.

Yeah, it does seem like you have one then. It can be corrected with bimax anyway, just discuss it with your surgeon

Bimax and composite bonding will revive you lmfao

Yes, in 2 years. Getting quite a few procedures throughout the next 2 years

Nice 😍 I’m guessing that’s post-op, with braces and all? Is your bimax covered by insurance?

I told the state that I’m extremely bullied, avoid social situations, and feel very uncomfortable with my appearance, so I got a referral for bimax and approval to do the surgery for aesthetic reasons jfl

I told the state that I’m extremely bullied, avoid social situations, and feel very uncomfortable with my appearance, so I got a referral for bimax and approval to do the surgery for aesthetic reasons jfl
Yeah, but I’m paying out of pocket. Also getting other procedures like sliding genioplasty, lateral commisuroplasty, rhinoplasty, chin implant, and upper blepharoplasty. Only ones that are most annoying to recover from are bimax and genio.

Also no fucking way that worked 😂😂

Yeah I might but I’m not sure if I need it. I need to measure my palate properly first. Thank you for the suggestion

Holy fuck, good luck with all the surgeries! I’ll probably have to sell the house to afford them.
