(I'VE ALREADY INFORMED MY ORTHO ABOUT ALL OF THESE SYMPTOMS, THIS IS JUST FOR DISCUSSION PURPOSES)

I live in a country where bimax is covered if you meet the skeletal criteria, and I’m getting a consultation + ceph early next year. I wanted to share my symptoms (excluding aesthetic ones) and hear what you guys think about whether this could be Class II / retrognathia-related.

Symptoms I experience:
  • TMJ clicking/popping and a soft cartilage noise when opening wide
  • A static pressure or “ear-fullness” feeling when I bite down
  • A weird static/crackling sound in one or both ears when my jaw is in my natural bite
  • That noise disappears instantly if I move my lower jaw slightly forward
  • General feeling of TMJ instability — the joints feel “better aligned” when the mandible is a bit advanced
  • Snoring
  • Difficulty holding the tongue comfortably on the posterior third of the palate. (I have issues with the front too, but that's due to a narrow palate, not recession)
  • Occasional airway restriction sensations
  • Lifelong ear–nose–throat issues, especially constantly blocked ears

The ear/TMJ thing (in more detail):
When I wear headphones, or when I lie on my side on a pillow, I get this consistent static/crackling noise inside my ear. It feels like pressure or rubbing. It only happens when my jaw is in its natural bite.
If I slightly advance the mandible, the noise disappears immediately.
Given that I’ve had blocked ears most of my life (and a lot of ear infections as a kid), I’m starting to suspect that the jaw position might be contributing to pressure on the ear structures or Eustachian tube.

I was treated for Class II at age 10–12. I had around 7.5 mm overjet at the time and wore braces + elastics for two years. The elastics were mainly upper canines to lower molars (don’t remember exactly). My teeth look fine now, but I suspect the skeletal component might not have been fully corrected.

Obviously the ceph will ultimately decide whether I qualify for surgery, but I’m curious what you guys think based on these symptoms.
The reason why I'm telling you about my functional symptoms only is because that's what the ortho will be evaluating, they do not care about aesthetics.

If my ceph concludes that I am not recessed enough for orthognatic surgery I will immediately start saving up for genio

Revisit this post after your consult

I’m invested bc your tmj sounds very similar to mine, have you ever tried Masseter Botox for it?

Never tried masseter botox. I've lived with these TMJ problems my entire life, so it's not like I need an accute band-aid treatment.

How I'm thinking is that I'd rather want to fix the structural/skeletal problem first and foremost
