The mandible already had asymmetrical width to begin with.
Not sure how BSSO could fix this but the mandible definitely looks under-advanced.

Someone correct me if I'm wrong about this but I don't think BSSO actually address jaw width at all.

The lower jaw doesn't seem to be canted or crooked.

It does if it is custom plates but to some extent, so it wouldn't address his problem really well, but there's risk of condyle problems and relapse.

Based on what I know about BSSO, advancement of the the mandible can give the illusion of greater width.
I reviewed the guy's photos and it looks to me like the ramus on the left side is longer than the right.
However, he did also receive asymmetrical advancement of the mandible on the left vs right sides.
Weird case.

However, he did also receive asymmetrical advancement of the mandible on the left vs right sides.

Edit: I was wrong.

It looked again at his case. There was greater BSSO advancement of the left side vs right.
Regardless, the mandible was clearly under-advanced overall + he needs a genioplasty.

You can torque the condyles up to around 5 mm each side. And when you advance the jaw it will be heavier weight for the masseter muscle making them generally bigger which do increase width most of the time. As you can see this guy clearly has more narrow jaw after becuase his massters lost size due to recovery and not eating hard foods. So he will gain back some width

His assymetry is better in the after, but like someone said ramus height and overall muscle assymetries cant be adressed. When you have a smaller ramus bone on one side your jaw muscle will generally be shorter and smaller on that side. And that cant really be adressed. Just like one person usually has a bigger bicep on one side and so fourth. But yea the advancements are incredibly underadvancements. Sad too see bluepilled doctors doing such a invasive surgery on probably greater amount of time because of lack of experience, but with extremly bad outcome because of underadvancements if you compare to a bp surgeon doing many surgeries a year

Just because there are worse cases than this doesn’t mean this is not brutal.

I mean a botch is when you get worse after surgery or get health issues. This is just a surgery not going to well. He probably got it for free so it is not a botch, he got better symmetry if you go on bone level

He got more BSSO advancement in the left vs right side as well.
Surgeon confirmed this.

Yea I looked at the scans, its more symmetrical after. When you got that much assymetry before its impossible to make it totally symmetrical. The biggest looksmin for him is the masseter muscle being smaller from the front

True. It thought the same as well. Issues like ramus height and jaw width also play into it - aspects which aren’t directly addressed by BSSO.

Regardless, he was clearly under advanced.

Yes, so many here also drastically over estimate what bimax can achieve.

In cases of asymmetry unfortunately, it often extends beyond the jaws.

yea this guys failos were long phil and nct, nct could have been adressed with more ccw snd bigger movements changing head tilt. and a big genio would have tighten the jawline, making his jaw a lot better

I know some top surgeons do bone shaving though to address this, like Raffaini.

Haha this guy has no bone to shave, but raffaini also does fat grafts to make ramus and stuff appear longer and wider. But i dont know how long lasting.

This is the the thing when your insurance covers the surgery, you can only go to insurance approved surgeons

And in most cases their only goal is to make it functional, they don't care about aesthetics


And even some private surgeons are not aesthetically focused either, this is why it's so important to do your research about the clinics and surgeons

Nah look at the scan its the mass of the bone that is less on one side, look at the antegonial notch on the shorter ramus side. And the bone is a reflection of the muscles being smaller on that side which is why we see one side being wider and more devloped.

its also less obv to see the left assymetry because you see the other ramus behind, another sign that side is shorter compared to right where you dont see the other ramus behind

Interestingly, he claims that pre op he thought the right side of his jaw was more developed than the left.

Any thoughts on why this is?

yea i mean this is a pattern i have been seeing, his orbits miight be wider and more square. but they are more compact(smaller) on the other side. and the zygo may appear to sit higher. and also the mandible usually is longer in length on the side which has a shorter ramus. i think it has to do with bonemass being distrubuted unequally rather then one side having less then the other side.

and that also seems to be the case when you look at the right side getting more mandible length

probably because the side which was longer had an disadvantage with being less vertical, but being longer made it better before either way, when right side became longer it has both vertical and lenght which makes it more defined because of both factors

The mandible was under-advanced but the frontal pic looks marginally better post op.
This is a challenging case because of underlying asymmetries.
