The buccal fat pad sits deep in your mid-cheek, between the buccinator and masseter muscles. it’s not the same as normal facial fat that comes and goes with body fat changes. Even when you’re lean, you can still have full cheeks because buccal fat doesn’t shrink much. It’s basically a gliding pad that helps your facial muscles move smoothly when you chew or talk.
What the Procedure Does
Buccal fat removal is a cosmetic surgery where a surgeon removes part of that fat pad from inside your mouth.
You end up with a leaner, sharper midface with more visible cheekbones, less roundness. However, once removed it's permanent. You can get fat grafting to sort of make it more normal, but it will never return fully to normal

When done correctly and with great final results it will give a more sculpted midface, better cheekbone definition and hollow cheeks.
However When done incorrectly it will give sunken cheeks, early aging look and or an asymmetrical appearance.

Humans naturally lose facial volume as we age, which is why it’s important to preserve natural fat stores when we’re young, rather than removing them. Without buccal fat, people in their 40s and 50s have sallow, sunken, and more mature-looking faces. And once the fat pads are gone, they can’t be replaced.
In your 50 or 60's, the muscles and skin around the buccal fat pad (which is now removed - it's purpose was to keep check around the area) will start to fill out the void. Basically making you look way older, and our primary goal is to keep our youth. It will ascend you in your 20's but when you hit your 30 or 40's you could end up looking 10-15 years older than what you really are.
This is also why considering a partial removal would be better in most cases.
