I have a classic downward grown flat asian maxilla failo with an underbite, leading to a very unappealing side profile. As shown on my sagittal CT, I've also had very rough TMJ issues for years now (very little room for the joint) and I experience a lot of flareups and clicking.

Now, the problem is that asians also have an extremely tricky base for bimax. Really easy to end up looking like a dog if there's no actual room for advancement.


I do have a decent front profile (zygos, eyeshape, bigonial width) especially for an asian and I also want to be careful not to sacrifice frontal balance or any ratios in order to improve the side profile. Midface shortening + jaw work would definitely help though.

I've already consulted with Pagnoni regarding side projection and he recommended ortho followed by Trimax+Infras. I also plan to consult additional surgeons as well including Ramieri and other U.S. based ones.

At the moment, I believe the best way to go about this would be to decompensate my incisors through orthodontics to allow for more advancement and then CCW bimax + genio. I don't have a gummy smile so it'd likely be a posterior downgraft rather than anterior impaction.

I've also considered infras + rhino too but I want to address the trimax first. And my main hesitation with infras is, I've got a pretty good eye shape and don't know how it might be affected with them. I do have shit orbital structure though and my eyes are mainly carried through pfl and eyeshape.

Wanted to hear forum's thoughts on this and what else I could possibly get done.

Get commisuroplasty, u need wider lips/ mouth opening, one of your biggest failos.

It’s a rare surgery, but worth a try.

But yeah get DJS, don’t bother with Ramieri cus he is booked up and he is hiking prices now

from who? not a single plastic surgeon is going to touch the idea of a commisuroplasty with a ten foot pole.

I don't believe a bimax is absolutely necessary in your case. However, the only guidance I can give is to find good surgeons in East Asian countries. I would especially recommend Korea and Singapore. Do not go to Western doctors.

I agree on my lips and it sucks because it contrasts with my wide palate.

Honestly haven't done any research on the procedure would definitely need to look into it and availability.

The entire trio is hiking up right now. Pag is quoting for 50k+ on implants alone. I've heard Ramieri has the best rep though but yeah he's booked crazy atm.

That's interesting why do you think bimax isn't necessary in my case?

Honestly don't really know about getting procedures in east asian countries. Just a completely different realm of beauty standards. Do you recommend any surgeons you think I should consult?

I've actually heard surprisingly good things about some Western surgeons especially kasey li on asian patients and derek steinbacher.

Trimax is going to be the way to go for you.
i guess with dog look you mean recessed midface because your jaws got advanced bud midface stays the same.
To avoid this you can consider infra malar implants that will basically make it look like you did a lf2.
With implants you really got to make sure you get a great designer or they look uncanny.
Also how much advancement are you trying to get?
