Can you actually ascend as a ltn?

Im wondering cause I don't think Ive seen one ascension when the guy is reccesed or boneless before. Its always just losing weight or fixing skin.Everybody talks so much about surgeries but will they even help if you don't have the genetic base. I don't think i Saw 1 transformation of a genetic LTN using surgeries. Give me a example if I'm wrong.
