Yea correct, that is why most oculoplastics club llrr with graft under the term canthoplasty, and I'd say probably more than half of people with apparent NCT have it because of lateral lid sag, not actual lateral canthus position. I think you only need actual lateral canthus repositioning if you have proper, severe NCT, especially if you have this but no scleral show. Let's say you have neutral or very mildly negative + scleral show and you want apparent PCT, it is probably better to just slope the lid contour up via graft to create this look than to additionally reposition the lateral canthus.
