Unless you have a syndrome tier palate don't get mse, and chances are if your palate is narrow you need bimax. The best way to fix a narrow palate and slightly add nasal volume for breathing is to get a multi segment lefort 1 osteotomy, Basically what that does is unlike a regular lefort 1 the surgeon splits the maxilla into 3 pieces which then he can use to widen your narrow ass palate. Looks aside if your breathing is shit (which I know it is) he'll also add bone graft to increase the nasal floor but unlike mse it won't give you a tyrone nose. Furthermore Mse only projects the zygoes laterally which gives people a "moonfaced" appearance, what you see with many chads that have "prominent zygoes" is that their zygoes are projected anteriorly, and forward. Also mse causes terrible asymmetry which sometimes lefort 1 can't even fix and there are cases where one cheekbone is more fucked than the other.

You could choose sarpe over marpe but even still its pretty expensive and chances are that if your palate is narrow your mandible and maxilla are retruded

Overall any adult expansion in my opinion is not only barbaric, but also a looksmin, Anything that isn't invasive and doesn't land you in the hospital for a night Is cope

So moonface= more prominent zygos?

I agree but your info is outdated, have you not seen the fme results on reddit? 0 issues w symmetry or anterior focused expansion. Yes expansion in itself is still is a looksmin 9 times out of 10. That being said, it can be tolerable depending on the case; crossbite, etc.

how much can the maxilla be expanded with multi segment lf1? what's the maximum

only by a few mms. you are limited by how much your palatal mucosa can stretch

Will multisegment bimax widen the cheekbones/face at all?

Also will it widening the palate cause a more fat, less "LEAN" defined model face/less hollow cheeks??

i have cleft palate what do you guys recommend

I basically agree with all of your points. Personally, I'm opting for just jaw surgery over marpe because I feel marpe would hurt my aesthetics, and my nasal breathing is already decent.

I don't really see the point of sarpe as opposed to segmental lefort 1 expansion. You get all of the downsides of marpe with none of the control and aesthetic benefits of a segmental.

1. It depends on the steepness of your palate. Some people have a relatively flat palate, others have one arched at a steep angle. The steeper your palate, the better candidate you are for a multisegment lefort. If your palate is narrow and flat, you'll get a better result from a max expander.

2. A carefully monitored custom MARPE carries low asymmetry risk just because the process can be stopped if things start drifting off track. That said, all current evidence I've see suggests that FME is the FAR superior option for achieving symmetrical expansion. And if the patient is planning a bimax afterwards, any residual asymmetry could potentially be corrected then.

3. In terms of breathing/nasal airway an expander also delivers a superior result - if this is what a person cares about.

4. Multi segment has a greater risk of relapse (people often overlook this tbh). It also won't enhance your zygos. It's also a harder surgery than a normal lefort.

5. Even a multi segment will still expand the width of the nose... and the expansion is more than a normal 1-piece lefort 1. I know people say just get a alar cinch etc but from what I've seen the results are actually mixed at how effective it is at stopping nasal widening.

For me I've made up my mind, I'll be getting either custom marpe monitored every few days or I'll be flying out to get FME done. The swing factor for me is the chance to get extra lateral expansion of my zygos... they already pop out a little bit so an extra 2mm of projection on each side would be perfect.
