He had a recessed midface, overprojected mandible and lower teeth, and recessed flat non existent chin in the before. That's what you're actually looking at.

In the after, he has a overprojected bimax cut, pushing forward the upper teeth and leaving the midface behind, a balanced mandible, and a flat and recessed non existent chin.

He may be swollen still in the midface.

He's fat, probably stupid, and has no chin. He needs to lose weight, get a massive genioplasty like 12mm and lose the swelling and he would look completely different.
