Disappointed with my 2D surgical planning BIMAX

I had my 2D surgical planning today with both CCW and CW options in dolphin imaging, and I'm honestly quite disappointed with how the simulation looks.


I'm having the surgery privately. My surgeon said that the movements in the current plan are considered safe, but that advancing the mandible further may no longer be safe.


The current plan is approximately:

• Mandibular advancement: 20–20.5 mm (with genio 4mm)
• Maxillary advancement: 8 mm
• Upper jaw CCW rotation: around 8–11°

My surgeon told me that the result should look better once we do the 3D planning, but looking at the 2D simulation I honestly feel like there is almost no aesthetic improvement.


What worries me is that from the side, I still feel that my profile looks very similar to how it looks now. I was expecting a more noticeable improvement in the profile based on my frontal facial appearance.


In the simulation, I still feel like I have a very bird-like profile. I still look like Woody Woodpecker.


For those who have had double-jaw surgery: did your 2D simulation look worse compared with your eventual 3D plan or actual result?


It looks like my jaw isn't moving forward, but rather downwards.


I've also discussed zygomatic and jaw angle implants with the same surgeon, but only as a separate stage once when bimax has fully healed, they're not part of this plan.

You are delusional if you think the diff of bimax in your case will be that big. Firstly your side isnt that bad to begin with. So the diff is not possible to be that big. Also you need some kind of submentql surgery if you want very good jaw from the side. Also you just seem delusional, bimax isnt some saviour surgery if you arent borderline deformed

Could be a downgraft causing that. Speak to the surgeon

These soft tissue predictions are ass. It's a known problem most good surgeons acknowledge.

You should scroll through r/jawsurgery and read lots of cases with photos, you'll see the outcome can vary a lot compared to the soft tissue simulation

There are limits to movements for a reason. It's not just safety. For example, the surgeon has to work with your occlusal plane. Or how much your upper lip can tolerate forward movement.

You're right to be cautious. You can always get a second opinion from another surgeon, consultations are relatively cheap

I have visible retrogenia and a flat face, and you say my profile isn't bad. People on this forum want surgery even when they don't have an orthognathic deformity, while I have an obvious skeletal defect.

Just an issue with the simulation and I personally don't think you have a noticeable bird face in the afters anyway.

Also 20mm advancement wow, you dont see that much very often.
